EMPLOYEE INFORMATION PAPERWORK
A with many things in life there is always paperwork that needs to be completed. This page gives a brief overview of and links too forms you will be required to fill as part of the employment process.
 
Registration Form: This expands on the information you supplied when you registered on-line.
 
Induction Form: All employees are required to complete a safety induction prior to taking on a new role. It should also be noted that when required employees must also complete a site-specific induction provided by the client including any specific client training requirements.
 
Health Assessment Form: As part of a candidate risk assessment all potential employees are required to complete a comprehensive Health Assessment Questionnaire.
Privacy Statement & Consent Form: To be signed by all potential employees.
Terms & Conditions of Employment: This document represents AVT Resources Standard Terms & Conditions and must be agreed to when signing your employment agreement.
 
Timesheets: Timesheets can be completed online after your login or by emailing/faxing a downloaded hard copy.
